Place of Service Question

If one is dispensing diabetic therapeutic shoes to a patient in a nursing facility who has resided there greater than 100 days(i.e., non-skilled), what would one use as place of service?

Would it be nursing facility or home? I am aware that if the patient comes to the office to have shoes dispensed, we should bill for diabetic shoes as home...but what if they live in a nursing (non-skilled) facility?
